This achievement is unlocked by collecting all the rubbings from the 30 ancestral tombs located in Vvardenfell and then completing the quest 'Lost Library'.
To begin this quest, go to the Library of Vivec in Vivec City and talk to Librarian Bradyn. The Library of Vivec is located on the lower levels of the structure with the Vivec Temple Wayshrine. Once you've spoken to Librarian Bradyn you're free to travel around Vvardenfell and locate the ancestral tombs. You can find all 30 of the tombs on the map below:
The ancestral tombs containing the rubbings that you can acquire by interacting with them (using the
button). The ancestral tombs are small pillars typically located near a glowing red light. They look like this:
If you're having particular difficulty locating the ancestral tombs, I would recommend cross-referencing with the guide below:

A guide to locating solving the puzzles involved in the quest can be found here for those who require it:
http://teso.mmorpg-life.com/morrowind-lost-library-quest-anc...Once you're done with the quest, speak to Librarian Bradyn one final time and you should unlock the 'Ancestral Tomb Hunter' achievement as well as earning a model-scale replica of Vvardenfell to use for your in-game house.
**********************************************************
Please note that the rubbings from the ancestral tombs won't have registered as being collected in the in-game 'Ancestral Tomb Hunter' achievement tracker (you can access this by pressing the
button, scrolling down to 'Journal', then 'Achievements' and then 'Morrowind').
In order for the rubbings to have registered in the in-game achievement tracker, you need to speak to Librarian Bradyn
after you've collected a rubbing. This will then remove all collected rubbings from your inventory.
I would recommend keeping a list of the tomb names and crossing them off as you go along. The names and map reference numbers of the ancestral tombs are:
